Community Information Stand in Isleworth

Date: Monday, 2 September **Time: **10:00 AM - 12:00 Noon Location: Isleworth Leisure Centre and Library, Twickenham Road, Isleworth, TW7 7EU

At the start of the week, **Healthy Hounslow **will be joining the **Community Information Team **on The Gazebo, Adult Social Care (ASC) Information ‘pop up’ shop. They will also be joined by Harpreet Rai, the Falls Prevention Manager.

What's on offer?

Healthy Hounslow:

• information, advice, and signposting on healthy lifestyles.

• guidance on where to get your blood pressure checked.

• tips and advice on monitoring your blood pressure.

Falls Prevention Service:

• information on falls prevention.

• Clinical Service information (West London Trust).

• information on Community Services and Support.

• onward referrals if needed.

The Adult Social Care Pop-Up Info Shop (Gazebo) will be able to provide information and signposting on various topics, including:

• care and support options for those struggling at home due to age, illness, or disability.

• what help is available if you look after someone as their carer

• how to use CarePlace, the council’s online advice and information directory

• activities and courses available in the borough

• employment and training opportunities

• what help is available if you want to lose weight, give up smoking or make healthier choices

• other organisations, charities and groups that can offer support in the borough.
