What is this service?

Who is eligible?

To be eligible to use this service you need to be:

  • Aged 18 or over

  • Living in the London Borough of Hounslow

  • Have a disability as defined by the Equality Act 2010 (a physical, mental, emotional, sensory, or developmental condition which has a substantial and long-term impact on your daily life)

  • At risk of becoming isolated or losing contact with services

  • Ready to engage with support and signposting with other professionals and services.

The Prevention, Transition and Development service is not intended to replace ongoing floating support or personal assistance needs. For some areas of need it may be necessary to seek specialised professional support.

We often work in partnership with Adult Social Care (ASC) to respond to safeguarding and wellbeing concerns. The level of involvement will vary according to individual circumstances and existing support.
