What is this service?

Learning disabilities support

At Bluebird Care, we support you to live the life you want, where each day can be different. Our staff recognise each person as an individual with a range of unique personal interests and aspirations. By focusing on your unique strengths and abilities, our aim is to help you achieve your personal goals. We develop a personalised support plan with you, so you can receive your care in the community where you live.

Alzheimer's disease care

Bluebird Care offers a range of services to support people with Alzheimer’s so they are able to be as independent as possible and live the lives they choose. We believe in working in partnership with the customer, their family and other care professionals to provide personalised care and support.

Physical disabilities care

We recognise each person as an individual with different interests and aspirations. By focusing on your unique strengths and abilities, we can support you to achieve your personal goals.

Choosing Bluebird Care means care and support to:

  • Manage your responsibilities - go to work and keep on top of day-to-day living tasks

  • Keep connected - meet friends and family,go to school, college or clubs, pursue a favourite hobby

  • Keep fit and healthy - attend medical appointments, eat well or go to a fitness centre

  • Stay independent - take a trip to town to do the shopping and pay bills

  • Maintain your daily routine - get up and go to bed

  • Keep your home clean and tidy – help with domestic chores like washing and ironing

End of life care

We can design an end of life care plan with you and, where you wish it, your family or friends. Our domiciliary care service will then be tailored to meet your specific needs and preferences. We value and respect each person’s spirituality and their right to dignified care at this time of their lives.

Our team will work with health professionals and other people who are involved in your care and support to manage your symptoms, maintain your dignity and keep you as comfortable as possible.

Palliative care

Our fully trained care assistants offer sensitive care and support to help you and your loved ones through the difficult times.

We agree a care plan that is tailored to your personal needs and choices. Our dedicated care assistants support our customers to be as comfortable as possible in the familiar surroundings of their own home.

Reablement services

Following illness or injury, you may need care and support to help you regain your independence in your home. We support our customers who require reablement services, for example, following a stay in hospital when you require support through your transition from hospital to your own home.

We provide focus on your development of robust coping skills for dealing with emotions that reablement can bring out during recovery, aiming for you to get your confidence and independence back as quickly as possible.
