What is Physiotherapy and Home Assessment Support?
Special People provides physiotherapy and home assessments for children and adults who need support with mobility, balance, rehabilitation, transfers or everyday movement.
Our clinical team considers the person’s health, abilities, goals, daily routines and home environment. We then provide practical recommendations designed to improve safety, confidence and independence.
Who is the service for?
The service may be suitable for people who are:
Recovering after surgery, illness or a hospital stay
Living with stroke, Parkinson’s disease, multiple sclerosis or an acquired brain injury
Experiencing reduced mobility, weakness, pain or balance difficulties
At risk of falling or recovering after a fall
Managing arthritis or another long-term physical condition
Using mobility or moving-and-handling equipment
Receiving support at home or in supported living
Seeking to maintain their mobility and independence
Suitability is confirmed following a review of the person’s needs.
What support is available?
Depending on the assessment, support may include:
Functional movement and mobility assessments
Falls-risk assessments and prevention advice
Post-operative or post-hospital rehabilitation
Personalised strength and balance exercises
Moving-and-handling assessments
Safer-transfer guidance
Home environment and accessibility assessments
Advice about mobility aids, seating or other equipment
Training and guidance for relatives or care staff
Written recommendations and progress reviews
Assessment and rehabilitation
During the assessment, the clinician may look at how the person walks, transfers, uses stairs, gets in and out of bed or chairs, and completes everyday activities.
Where appropriate, a personalised rehabilitation or exercise programme can be developed. The programme will reflect the person’s current abilities and agreed goals, such as improving strength, reducing falls risk or completing daily activities with less assistance.
Progress can be reviewed and the plan adjusted as the person’s needs change.
Home safety, transfers and equipment
The clinician can assess how the home environment affects the person’s safety and mobility. This may include entrances, stairs, flooring, lighting, furniture layout and access to bedrooms or bathrooms.
We can also assess how the person is supported with transfers, repositioning and mobility equipment. Practical guidance may be provided to relatives and care staff to promote safer practice and reduce avoidable risks.
Where equipment or adaptations may be helpful, recommendations can be made. Some items may require further assessment or approval from an occupational therapist, NHS service, local authority or equipment provider.
Reports and professional coordination
A written report may be provided following the assessment. With the person’s consent, it can be shared with relatives, care providers, commissioners or other professionals involved in their support.
Where appropriate, we can communicate with physiotherapists, occupational therapists, GPs, nurses, social workers, care coordinators and hospital teams to support a coordinated approach.
Referrals
We welcome enquiries from individuals and families, local authorities, NHS teams, hospital discharge services, care providers and other health or social care professionals. We also accept enquiries from people who are self-funding or using a Personal Health Budget.
All services are subject to assessment, clinical suitability and availability.

Support is planned around each person’s individual needs, preferences, communication and goals. Services may be delivered in the person’s home or in the community. Please contact us to discuss eligibility, availability, funding and the areas we cover.
We keep individuals, families and relevant professionals informed throughout the referral and assessment process. Once support begins, the care team will communicate regularly and review the support provided when needs or circumstances change.
